That is a facilitation problem, not a template problem, which is exactly why the template is free and the facilitation is not yet offered.

A candidate service is usually a marketing state. Here it is a list. Each item below is genuinely undecided, and publishing a price before deciding them would be selling a shape rather than a service.
Duration and format. Whether this is a single facilitated session, a session plus a written pass, or a short sequence across a decision that is still moving. Each implies a different price and a different commitment from the organization, and guessing at it now would produce a number nobody could stand behind.
OpenWhat is delivered, and by whom. A record can be facilitated and typed by the organization, or drafted afterwards by RN, or both. Drafting on someone's behalf raises a question about whose reasoning is being preserved, and that question is not settled.
OpenPrivacy handling. A consequential decision is described with material that is frequently confidential. There is no confidential intake route on this site, and one would have to exist — with retention, deletion, and a written agreement — before a session could responsibly be booked.
OpenSupport after the session. Whether a review date months later is part of the service or the end of it. A decision record whose review trigger nobody honours is an artifact, not a control.
OpenThe shape of the artifact. This is settled. The record covers the decision and its accountable owner, the alternatives, evidence and assumptions, the affected-party impacts and unresolved questions, and the next action with stop conditions and review triggers.
